Space Quiz

With quizzes being among the most engaging types of forms I wanted to try a few tactics unique to the format.
• Logic Jumps are used halfway through to offer shortcuts to the end (you may find one Logic Jump gap due to being limited to 3 jumps with the Typeform Essentials Plan)
• Combinations of Multiple Choice, Short Answer, Picture Choice, and Yes/No questions used throughout
• Score is calculated by adding 1 point for every correct answer
• End screen reports "You got X correct answers" to accommodate Logic Jumps skipping questions while keeping the score relevant.
• With the Typeform Professional plan I would be able to utilize unlimited Logic Jumps to create instant correct/incorrect feedback and more.

Leak Repair Request

Created to automate incoming requests for service work, this form reduces phone calls and gathers qualifying information for the service department.
• Uses a Logic Jump if the participant opts for "No" on question #4 to reduce the 13 question form down to only 5 questions.
• Recall tags are used to personalize multiple questions including the final End screen
• Picture Choice questions increase continuance rates (approximately 5%) - like question #5

Daily Hours

This form allows employees to submit their hours worked for the day, each day, via the web. This benefits managers with less paperwork, quicker gathering of this critical information, and more accurate data.
• Designed to allow for up to 3 jobs worked using only 2 Logic Jumps (Typeform Essentials Plan) to skip unneeded fields.
• Aggregates data to a Google Sheets shared spreadsheet for access by managers and payroll team
• Notifies manager of submission instantly via email.

Appointment Lead Time

This form utilizes Typeform's Calculation and Recall features to customize the lead time copy.
• Cut & Color "[3] weeks"
• Just a Cut "[2] weeks"
• Kids Cut "[1] week(s)"

Workbench Order

Here is an early sample form I created to point a craigslist listing to. It's a 4 question "order form" for a custom made workbench. My goal was to keep the total number of questions to a minimum while giving me enough information to follow up.
• I applied the 'Desk Space' template from within Typeform to quickly and easily give it a more polished feel.
• Originally embedded into a separate webpage there is no end card as any additional information or next steps were already on the page.
